Step 2: Complete the registration contact form. It is critical that coaches have up-to-date phone and email information for BOTH players and parents.
Step 3: Download the BHS Athletic Participation Card. Print out the form, get your physical, and bring to the first day of tryouts (see home page for details). Every player needs a completed BHS Athletic Participation Card to participate in tryouts.
Step 4: Download BHS Girls Lacrosse, Inc. Waiver Form. Print the form, sign it and bring to Code Night.
Step 5: Download the LAX Code Night Registration Form. Print the form, select your uniform and equipment, have BOTH player and parent sign, and bring to Code Night with full payment (personal check).
Step 6: Attend Code Night at 6:30 pm, March 7, in the BHS Cafeteria. This is a mandatory meeting for BOTH players and parents. Remember to bring the US Lacrosse print out, BHS Girls Lacrosse, Inc. Waiver Form, and GLAX Code Night Registration Form and checkbook.
